Terms of Service

These Terms govern your use of the TriageX patient intake and triage platform, operated by Entrivo. Please read them carefully before using the platform.

1. About these terms

These Terms of Service ("Terms") govern your access to and use of TriageX, a patient intake and triage software platform operated by Entrivo ("we", "us", "our"). By signing up for, installing, or using TriageX, you ("Clinic", "you") agree to these Terms in full. If you do not agree, you must not use the platform. 2. What TriageX is — and what it is not

TriageX is a pre-appointment patient intake and triage tool. It is designed to: - Structure patient complaints before a clinic appointment - Screen for safety concerns and potential red flags - Guide patients toward an appropriate next step (such as booking) - Deliver structured intake information to clinic practitioners TriageX is not a clinical decision-making system. It does not diagnose medical conditions, recommend treatments, or replace clinical judgment. All clinical decisions remain with qualified healthcare practitioners employed or contracted by the Clinic. You must not present TriageX to patients as a diagnostic or medical advice service.

3. Who can use TriageX

TriageX is intended for use by registered healthcare clinics and practitioners, including but not limited to chiropractic, physiotherapy, osteopathy, and sports medicine practices. By agreeing to these Terms, you confirm that: - You are authorised to enter into this agreement on behalf of the Clinic - The Clinic is a legitimate healthcare provider operating in compliance with applicable professional regulations - You will only use TriageX for lawful purposes related to clinical intake and patient management

4. Your account and dashboard access

You are responsible for maintaining the confidentiality of your clinic dashboard login credentials. You must not share your login with unauthorised individuals. You agree to notify us immediately at hello@triagex.com if you suspect any unauthorised access to your account. We reserve the right to suspend or terminate accounts where there is evidence of misuse, breach of these Terms, or non-payment of applicable fees.

5. Patient data and your responsibilities

When patients complete a TriageX intake flow on your clinic website, they provide personal and health-related information. You (the Clinic) act as the data controller for that patient data. Entrivo acts as a data processor on your behalf. As data controller, you are responsible for: - Ensuring your clinic's privacy notice informs patients that a third-party intake tool is used - Complying with applicable data protection laws (including UK GDPR and EU GDPR where relevant) - Obtaining any necessary consents from patients before directing them to the intake flow - Handling patient data in accordance with your professional regulatory obligations You must not use TriageX in a way that would expose patient data to unauthorised parties or violate applicable data protection law.

6. Clinical content and red-flag logic

TriageX includes structured question banks and automated red-flag screening logic developed to support clinical intake workflows. This content is provided for intake assistance purposes only. We do not warrant that the clinical logic within TriageX will detect every possible red flag or safety concern. The red-flag screening built into TriageX is an aid, not a substitute for clinical assessment. Your practitioners remain fully responsible for clinical assessment, diagnosis, and treatment decisions. Do not rely on TriageX output as a clinical safety guarantee.

7. White-label and branding use

TriageX offers white-label customisation, allowing the platform to be branded to match your clinic's visual identity (colours, logo, typography). You may customise the appearance of TriageX within the parameters provided by the platform. You may not: - Remove or obscure any legally required notices - Use the TriageX brand name or Entrivo name in a way that implies endorsement - Misrepresent TriageX as a product you developed or own We reserve the right to approve how our platform is presented in particularly sensitive commercial contexts.

8. Fees and payment

Access to TriageX is subject to the fees agreed at the time of onboarding ("Subscription Fees"). Fees are billed monthly or annually as agreed. Subscription Fees are non-refundable except where required by applicable law or as expressly agreed in writing. We reserve the right to change Subscription Fees with 30 days' written notice. Continued use of TriageX after the notice period constitutes acceptance of the new fees. Non-payment of Subscription Fees may result in suspension of dashboard access and, after reasonable notice, termination of your account.

9. Acceptable use

You agree not to use TriageX to: - Collect or process patient data in an unlawful manner - Misrepresent the nature or purpose of the intake flow to patients - Transmit malicious code, spam, or any content that disrupts the platform - Attempt to reverse-engineer, copy, or resell the TriageX platform - Use the platform in a way that violates professional healthcare regulations We reserve the right to suspend access immediately if we reasonably believe these obligations are being breached.

10. Intellectual property

TriageX, including its software, clinical logic, question banks, design system, and all associated content, is the intellectual property of Entrivo. Nothing in these Terms transfers ownership of that intellectual property to you. You are granted a limited, non-exclusive, non-transferable licence to use TriageX for the purposes described in these Terms, for the duration of your active subscription. You retain ownership of your own clinic brand assets (logos, colours, copy) provided to us for white-label configuration.

11. Availability and service levels

We aim to maintain high availability of the TriageX platform. However, we do not guarantee uninterrupted access and are not liable for downtime caused by factors outside our reasonable control, including infrastructure outages, third-party service failures, or scheduled maintenance. Where we plan maintenance that may affect availability, we will provide reasonable advance notice where possible.

12. Limitation of liability

To the fullest extent permitted by law, Entrivo shall not be liable for: - Any clinical outcome, misdiagnosis, or harm arising from the use or misuse of TriageX - Indirect, incidental, or consequential losses arising from your use of the platform - Loss of revenue or business opportunity resulting from platform downtime or errors Our total liability to you in any 12-month period shall not exceed the Subscription Fees paid by you during that period. Nothing in these Terms excludes liability for fraud, death or personal injury caused by our negligence, or any other liability that cannot be excluded by law.

13. Termination

Either party may terminate these Terms with 30 days' written notice. We may terminate immediately if you breach these Terms in a material way and fail to remedy the breach within 14 days of written notice from us. On termination, your access to the clinic dashboard and the TriageX platform will be suspended. Patient submission data will be retained for 30 days following termination, after which it will be deleted, unless a different retention period is required by law.

14. Governing law

These Terms are governed by the laws of England and Wales. Any disputes arising under these Terms shall be subject to the exclusive jurisdiction of the courts of England and Wales.

15. Changes to these terms

We may update these Terms from time to time. When we do, we will update the "Last updated" date at the top of this page and notify clinic users directly of material changes. Continued use of TriageX following notification of updated Terms constitutes acceptance of the revised Terms.
